Web Account Request Form
Account Type
At UCO there are two types of web accounts.
The first type is a Site account. The second type is a Developer
account.
A site account contains the
information and reserves the space for a specific web site on the university
server. This type of account is not assigned to an individual person but
instead defines the site. It may have multiple developer accounts associated
with it. A site exists when a URL has been established for it.
A developer account
contains the information for the individual responsible for developing,
maintaining, designing, working on, etc., a web site. This type of account
is assigned to an individual. This type of account may be requested by a
contracting party's sponsor. The developer may be associated with multiple
site accounts. Each individual assigned to work on a site should submit a
developer account request.

Account Category
Site accounts fall into 3 categories. The
first is an Academic Interest site. The second is a Departmental
site. And, the third is a Student Organization site.
An Academic Interest site supports the
academic mission of the University. This category of site generally has a
tie to a specific course or course of study. The UCO Passport site is an
example of an Academic Interest site.
A Departmental site supports the
functional mission of the University. This category of site is generally
more administratively oriented. The UCO Department of Public Safety site is
an example of a Departmental site.
A Student Organization site supports
the activities of an active student organization. This category of site is
limited to Student Organizations that are chartered through the Campus Life
Office. The Fellowship of Christian Athletes is an example of a Student
Organization site.

Descriptive Name of Site
The descriptive name of the site may be as
simple as the identify of the academic interest (i.e. Faculty Enhancement
Center), the department name (i.e. Information Technology), or the student
organization name (President's Leadership Council).

Organizational Area
The organizational area is the College or
Administrative Division that governs the site. For Academic Interest sites,
this would be the College sponsoring the program. For Departmental sites,
this would be the Administrative Division of the department. For Student
Organization sites, this would be the Office of Student Affairs/Campus
Life.

Requested / Active URL
For new site accounts, enter the requested
URL. URLS are
provided only in the formats http://www.ucok.edu/site_name or http://site_name.ucok.edu.
If you need mutliple URL aliases for a single site, enter the primary URL
here and then request additional URLS in the comments field at the end of
the form. If you are requesting multiple URLs for multiple sites, please
request an individual account for each site. For developer accounts, enter
the URL of the site you will be maintaining. Please note that Student
Organizations and Faculty sites will be granted URLS in the format of http://www.ucok.edu/site_name.

Tool
UCO supports Microsoft FrontPage as the
primary developer tool and offers training for developers that will be using
it. Alternate tools such as Macromedia Dreamweaver may be used but will
have limited support.

Advanced Elements
UCO supports different advanced elements on
different servers. Other tools may be requested but may not be
available or supported. You will be notified by the web administrator if the
tool is not available for use.

UCO Username
This is the same as your UCONNECT or Network
Login name, i.e. jsmith23. It will be used to generate your developer
account username. For account requests of contracting companies with no
previous university affiliation, please enter "unknown".

UCO ID
This is your UCO ID number such as *87654321,
not your Social Security Number (SSN). It will be used to verify your UCO
Username. For account requests of contracting companies with no previous
university affiliation, please enter "unknown".

Position
Job title for UCO employees or office, i.e.
president, for student organization members.

Sponsor Information
A Sponsor is required for Student
Organizations and contracting party accounts.
For Student Organizations, this is generally
the faculty or staff sponsor that assists the organization on campus.
For contracting party accounts, this is
generally the manager responsible for establishing the contract.

Contracting Party Accounts
Contracting Party accounts are developer
accounts assigned to someone who is not currently an active student, faculty
or staff individual. Requests for these accounts must be made by the
contracting party sponsor. The request cannot be approved if submitted by
the contracting party. The request will be verified with the appropriate
academic or departmental area sponsor before username and passwords will be
issued.
The contracting party
company information and manager information is required.

Form Submission
When the form is completed
with the required information, it is submitted to UCO Information Technology
for action and a response page is displayed. If the account is approved,
the developer will be notified of their username and the URL via email. For
this reason, it is important to have a valid email address entered on the
form. If the form submission does not appear to happen, you may print the
form (or response page) and either mail or fax it to UCO Technology
Support.
